Chaos;Head's story is set in the year of 2008 in Shibuya and is centered around Takumi Nishijou, a high school student at the private Suimei Academy, and some strange events that have occurred recently in the Shibuya area. Two events have occurred so far and is being referenced by the media and the public at large as being New Generation events. The first event named "Group Dive" occurred on September 7th with five high school students jumping to their deaths off of a high tower. The second event occurred on September 19th wherein a male was found dead with a thirty-two week old fetus incised within his abdomen.

The story takes place in a city resembling Italy, known as Belmonte, with the technology levels resembling levels in the late 19th century. In this city there are human-sized mechanical dolls known as "Automata" who serve the people of the city.The protagonist, Romeo, is a taxi driver who was originally involved in a organization known as the "Olma Rossa." He finds an automata in the shape of a young woman, who appears to have amnesia. He names her Anna, they start living together. However, sometime later, the Olma Rossa begin to catch up to Romeo, and an organization composed of automata also appears; thus, Romeo must fight to protect his current life.

A spate of mafia assasinations occurs in America, the work of the mysterious organization named Inferno. "Phantom" is their most strongest and capable assassin, however a lone tourist witnesses Phantom's latest act of murder. He unwittingly exposes the top assassin's true identity, a young girl named Ein. Witnesses are usually killed, but the man is captured and taken to be brainwashed into "Zwei", Inferno's newest assassin, with no memory of who he is. From this point on, Zwei is drawn into a world of conspiracy, deceit, and violence.
